The Aged & Disabled Medicaid Waiver Program is a long-term alternative that provides services to assist eligible individuals to remain in their homes rather than receive nursing facility care. Services included under Aged & Disabled Medicaid Waiver are case management and homemaker services. Randolph County Senior Center has been state approved to provide these services.
Case Management - Screening, needs assessment and care planning assistance through the initial
ADW process is provided by a licensed social worker or registered nurse. For
eligible clients, the case management team will work closely with the
homemaker agency to provide appropriate care to the client in their home on an
ongoing basis.
Homemaker Services - Provide direct hands-on care to Aged & Disabled
Waiver clients in their home. Care includes personal care, meal preparation, light
housekeeping, shopping & errands. All care is under the direction of a registered
nurse and delivered by a trained staff of homemakers.
Personal Care Program is a medically oriented program that provides services to eligible Medicaid recipients in their homes. Care is carried out by trained homemakers under the supervision of registered nurses. Care may include: bathing, grooming, toileting, dressing, skin care, ambulation, ROM exercises, meal prep and feeding. Environmental assistance is also provided including: light housekeeping, laundry, dishwashing, grocery shopping and bill payment.
Private Pay Personal Care Program Provision of needed assistance to individuals in their place of residence according to physical needs and limitations. Care may include personal care, meal preparation, light housekeeping and companionship. Services are provided by trained homemakers for a set hourly fee. Initial intake and plan of care is by a licensed social worker or registered nurse.
Veteran In-Home Care - Provision of personal care needs, meal preparation and light housekeeping to veterans approved by the Veterans Administration for these services. The plan of care and assessment is by a registered nurse and services are provided by trained homemakers.
Lighthouse Program - Provides support in 4 areas:
Personal care, mobility, nutrition and environmental support. Eligibility criteria: 60+ years of age, medically eligible based on a functional evaluation by the senior aging program's RN and financially eligible, determined by a sliding fee schedule.
Services are provided by trained staff of homemakers.
